Phlebotomy-Technical Certificate
The Phlebotomy Technician (PT) program is designed to provide the knowledge, laboratory skill practice, and real world experience necessary for successful completion of a Phlebotomy Technician Certification Examination and employment as a Certified Phlebotomy Technician (CPT).

Phlebotomy Technician (PT) Special Program Requirements
1. Completion of a SAR-40 Physical Examination Report that includes documentation of:
a. Current basic physical examination indicating good health.
b. Current Tdap (tetanus, diptheria, +pertussis) immunization.
c. Current influenza (flu) immunization.
d. Complete hepatitis B immunization series or signed waiver.
e. Current negative tuberculin (TB) test.
f. Positive blood titers indicating Immunity to rubeola (measles), parotitis (mumps), rubella (German measles) and varicella (chicken pox).
g. Satisfactory criminal background check and level II fingerprint check.
A student who has been indicted, found guilty of, or entered a plea of no contest to a felony or any offense that would disqualify one for employment in healthcare will result in program disqualification.
h. Negative urine alcohol/drug screen.
A positive alcohol screen or positive drug screen for a medication not currently prescribed will result in program disqualification.
2. A grade of "C" or higher for HCP 0731 Phlebotomy Theory and HCP 0731L Phlebotomy Laboratory.
3. A grade of "Pass" for HCP 0742 Phlebotomy Clinical.
