Application and Admission Requirements

1. Completed PHSC application and application fee
2. Completed Paramedic Certificate application and limited access application fee
3. Official high school transcript indicating graduation and receipt of standard high school diploma or an official transcript of GED scores indicating receipt of a high school equivalency diploma
4. Official transcript from each U.S. college and university attended other than PHSC
5. Official scores from external agencies as appropriate for prerequisite courses (ex: AP, CLEP, IB, etc.)
6. Copy of current Florida EMT license
7. Valid placement test scores (ACT, ASSET, CPT, PERT, SAT) and completion of any college preparatory work in English or reading, if indicated by the placement scores. The mathematics score must be at the Elementary Algebra (MAT 0024) level or above or the applicant must complete Pre-Algebra (MAT 0012) with a grade of "C" or higher. Placement test scores for English, reading and/or mathematics may be exempted if the applicant has completed transferable, college-level coursework with a grade of "C" or higher in each of the areas listed.
8. Copy of current driver's license, (P.O. boxes will not be used for residency preference in the selection process). If a driver's license, or equivalent, is not submitted as proof of residency, the application will be processed as a nonresident application. Submit verification form of current EMT employment in Pasco or Hernando County. If a driver's license, or equivalent, or EMT verification form is not submitted as proof of residency or employment, the application will be processed as a nonresident application. 
9. Copy of current American Heart Association "Basic Life Support for Health Care Providers" CPR certification (Expiration date must extend through program completion. If certification expires prior to end of the program, PHSC course NCH 0050, American Heart Association "Basic Life Support for Health Care Providers" must be taken.)
10. Cumulative grade point average (GPA) of 2.0
11. Complete HSC 2531 - Medical Terminology  with a grade of "C" or higher
